Residential Support Counselors
Work directly with survivors of traumatic brain injury in a group home setting.  Responsibilities include supervising and assisting the consumers in activities of daily living skills such as personal care, social and community recreation, medication and health care issues, encouraging consumers to be as independent as possible; as well as transporting individuals to community resources.  Requirements:  HS Diploma/GED; clean valid NYS driver’s license. Full time/Part time/Per Diem – all shifts available.

 

 

Independent Living Skills Trainers (ILST)
These trainers provide one-to-one training to consumers with TBI based on an assessment of needs.  Trainers assess the skills and abilities of the consumer in managing their daily living activities including personal care, independent living skills, social and community recreation, medication and health care issues.  Requirements:  BA/BS plus 1 year of experience or HS Diploma and 2 years of experience providing functionally based assessments, developing a comprehensive treatment plan and teaching individuals with disabilities to be more functionally independent.  Flexible hours; Clean NYS driver’s license with own vehicle required.

 

 

Positive Behavioral Support and Intervention Services (PBIS)
Responsibilities include behavior observations, data collection, documentation and implementation of the behavior support plan.  Provide behavior intervention to consumers and train all staff using functional analysis designed to decrease challenging behaviors due to TBI. Excellent clinical opportunity.  Requirements:  BA/BS with 1 year of related experience working with people with traumatic brain injuries, other disabilities or behavioral difficulties.  Flexible hours including evenings; Clean NYS driver’s license with own vehicle required.

 

 

Community Integration Counseling (CIC)
Provide one-to-one counseling sessions to survivors of TBI and their families.  Assist the survivors to effectively manage the emotional difficulties associated with adjusting to & living in the community. Requirements:  MA in Psychology, Social Work, MSW or CRC with two years experience providing adjustment-related counseling to individuals with traumatic brain injuries and their families.  Suffolk/Nassau locations.  Flexible Schedules, Part Time, Evenings, Weekends.

 

 

Community Habilitation Counselors
Responsibilities include working with OPWDD/TBI survivors one-to-one in their homes or residential settings providing independent living skills assistance, escorting them into the community, and assisting them with completing their Activity of Daily Livings Skills (ADLS). Requirements: HS Diploma and prior experience working with people with disabilities required.  Clean NYS driver’s license with own vehicle required.

 

 

Assistant Residential Program Manager

High School Diploma with one year of experience in residential setting, Bachelors degree preferred.  Flexible hours with some weekend work required. This is a full time, on-call position.

 

 

 

Nursing Supervisor

NYS Licensed Registered Nurse. Under the supervision of the Director of Residential Services, the Nursing Supervisor will be responsible in ensuring that the best possible nursing care is provided to the Individuals we serve. Ensure that health care services are coordinated and implemented for survivors of TBI. Ensure that the nurses advocate for medical services; liaison with medical providers and family members; provide oversight and guidance to staff members with regards to medical issues; supervise AMAP staff members; review and ensure that medical documentation is completed in accordance with the state regulatory agencies. Shifts assigned may vary and change according to Agency needs. Responsible for the direct supervision of the nurses assigned to all sites. This is a 24-7 on call position.
